ALL-STATE CHORAL AUDITION REGISTRATION FORM

An audition fee of $10.00 per audition shall be remitted for each registrant. Retain a second copy for your records.  Students shall not be scheduled to audition unless registered with fees paid (no personal checks from students) or school purchase order attached. Fees are not refundable. If the school is more than 150 miles one way from the nearest Audition Center, the school may choose to send a taped audition. Follow tape audition procedures found in the official Handbook. Tapes should be sent to the Audition Site Chair one week prior to the audition date. Please read your Handbook for the latest revisions in the audition procedure.

Special requests for auditioning on a specific date and time must be made on this audition registration form at the time the audition registration form is submitted. All requests for special scheduling consideration must be requested through the Executive Director.

If a student auditions for vocal and on a string instrument, their preference must be declared by placing an asterisk (*) in front of their name beside the instrument or voice classification of choice. For example: *Gerheart, Don.

The late audition period is from September 21, 2008 through midnight September 27, 2008.
The late registration fee is $22 per audition.
No audition registrations will be accepted after midnight September 27, 2008.

By submitting this form, the school Principal certifies the students listed above are in grades 9-12 and are academically eligible to participate in the New Mexico Music Educators All-State Auditions and Festival in accordance with the New Mexico Activities Association.
