     Welcome to the NMMEA Southwest District Forms Page.  Forms listed below use the PDF file format. This particular file format allows you to view and print your forms electronically on most all major computer platforms. Click on the highlighted words Adobe Acrobat Reader to download this program if you don't already have it.  This software is free of charge and is required to view and print PDF files.

     NOTICE!   You have the option of submitting the entry forms online for Solo and Ensemble Festivals or Large Group Festivals (click on the highlighted words to bring up email forms) or use forms downloaded by Acrobat Reader to enter by fax.   Either way is acceptable.  If you choose to print and fax your forms, please complete the required spaces and submit them to Tony Montano (505) 527-6681 by fax.  Mr. Montano will process your entries.  If you choose to enter by email, all processing will be done automatically.  With either choice, print a copy of the "Bulletin."  You will need that information.   Download a copy of the appropriate judging form, duplicate as many as needed and bring them filled out to the festival you are entering.

 

Instructions for Retrieving Forms

To retrieve forms, please follow these simple steps:

  1. Click on the title of the form you wish to preview and print.

  2. To reconsider, click the "Reset" button or select another form.

  3. To view, click the "Preview" button. Acrobat Reader will launch and load the selected form.

  4. Print the form from Acrobat Reader.

  5. Click the "Back" button on your browser to return to the this page.
